Learning from Incdidents/Accidents - Why, Why, Why?

Accidents and "near-misses" are typically preventable. Identification of the "root cause" of an incident is key to identifying preventive measures and process improvements to enhance the safety of workers, communities, the environment and to ensure business continuity. The presentation will focus on the need for post-incident investigation and introduce the audience to various techniques used in high technology industries to perform such an investigation.
About The Speaker: Varun Gopalakrishna is an EHS Consultant at EORM, Inc., and has extensive experience in the areas of process hazard analysis for metallization and gas delivery technologies, facility design review, hazardous materials management and design for safety and environment.
